Schedule will not exceed 80 hours within a 2-week pay period. Services will be required on Federal holidays.

DUTIES OF THE CERTIFIED REGISTERED NURSE ANESTHETIST (CRNA):

Preoperative evaluation: Prepare and review case histories and obtain data through interviews, chart review, physical examination, and interdisciplinary specialty consultation for patients undergoing surgical procedures, labor analgesia, or anesthesia for non-invasive studies or exams.
Provide routine and emergent anesthesia care for infant through geriatric patient populations with American Society of Anesthesiologists (ASA) classification I VI.
Provide regional and/or general anesthesia care for the following subspecialties: general surgery, obstetrics, gynecology, interventional radiology, cardiology, pulmonary medicine, vascular, adult and pediatric dentistry, otolaryngology, neurosurgery, urology, ophthalmology, oral and maxillofacial surgery, orthopedics, plastic and reconstructive surgery, podiatry, and dermatology, as assigned by NCR MTFs.
Prepare and review case histories and obtain data through interviews of patients.
Provide routine and emergent anesthesia care for neonatal through geriatric patients with American Society of Anesthesiologists (ASA) classifications I - VI.
Provide regional and/or general anesthesia care for the following subspecialties: general surgery, obstetrics, gynecology, cardiothoracic, vascular, adult dentistry, pediatric dentistry, otolaryngology, neurosurgery, urology, ophthalmology, oral surgery, orthopedics, plastic surgery, podiatry, and dermatology.
Administer appropriate diagnostic and therapeutic nerve blocks in the treatment of acute and chronic pain.
Provide resuscitative care during a cardiopulmonary arrest, and sedation or general anesthetic care at remote locations.
Personally provide this care or he/she shall supervise the simultaneous delivery of anesthesia care by the student registered nurse anesthetist.
Oversee the activities of the recovery room and administer appropriate drug and respiratory therapy in the recovery room setting.
Implement the proper use of mechanical ventilators in the recovery room, and as necessary, in the intensive care units.
Interpret tests results and evaluate findings.
Work under the general professional direction of the Chief of Anesthesiology Services.
Provide consultation services for members of the MTF staff for purposes of specialty area training. As required, the contracted CRNA will serve in a teaching capacity to staff members assigned to the MTF and ambulatory clinics.
Maintain accurate clinical records and receiving consultations and laboratory work of specific patients. The CRNA shall complete medical charting within 72 hours of patient visit IAW with JC guidelines.

QUALIFICATIONS OF THE CERTIFIED REGISTERED NURSE ANESTHETIST (CRNA):

Bachelor s Degree of Nursing from a National League for Nursing (NLN) approved school;Master s Degree in Nurse Anesthesia from an American Association of Nurse Anesthetists (AANA) accredited school
Certified as a Registered Nurse Anesthetist from the American Association of Nurse Anesthetists (AANA) or the National Board of Certification and Recertification for Nurse Anesthetists (NBCRNA).
Within the preceding three (3) years in the specialty, a minimum of one (1) year clinical CRNA experience working independently while also performing regional anesthesia, to include: neuraxial and peripheral nerve blocks, such as spinals, epidurals (lumbar epidural analgesia), interscalene, femoral, sciatic, and other extremity nerve blocks, independent of supervision.
Must be competent and have experience in obstetric anesthesia (general endotracheal, epidural, and subarachnoid blocks).
Must be competent with performing subarachnoid or epidural anesthesia.
Current ACLS and BLS
Current, unrestricted license valid in the state of Virginia.

Contractor shall have the ability to perform reassessment and management for all patients and per policy. This includes neonate, pediatric, geriatric, and the general population.

Demonstrates knowledge and skill to develop and implement policies and procedures to manage patients.
Demonstrates knowledge of the principles of growth and development over the life span and the skills necessary to provide age appropriate care to the patient population served.
Able to interpret data about the patient's status in order to identify each patients age specific needs and provide care needed by the patient group.
Ability to develop and revise plan of care as indicated by the patient's response to treatment and evaluate overall plan for effectiveness.
Prescribes medications, therapies, labs/ X-rays test, etc as indicated.
Assist with prescriptions for medication and supplements as appropriate.

The Contractor shall educate the patient and family regarding pain management.
Coordinates surgical procedures for team surgeons as indicated and conduct postoperative discharge follow-up phone calls within 48-72 hours of discharge to assess for medication / vitamin compliance fluid intake / diet toleration, possible dehydration and need for IV fluids. Comfortable caring for patients that are critically ill, who may require ventilator support, vasopressors, and invasive monitoring.
Demonstrates ability to perform treatments and provide services to level licensure.
Serves as a mentor to nursing, medical, and physical therapy students as well as residents and nurse practitioner students.
Formulates a teaching plan based upon identified learning needs and evaluates effectiveness of learning, family is included in teaching as appropriate.
Provides continuous long-term post-operative care and ongoing education to all patients undergoing surgery. Provides pre and post-op assessments and patient/family education.
Knowledgeable of medications and their correct administration based on age of the patient and clinical condition.
The Contractor shall be the resource person to physicians, nursing and other hospital staff.
Serves as mentor to the Registered Nurses on staff and within the facility;communicates appropriately and clearly to nurse managers, nursing staff and physicians.
Consult other healthcare professionals/departments as appropriate to provide interdisciplinary approach to the patient's needs.
Complete thorough timely documentation for visits and treatment plans for the management of patients through in-depth progress notes and dictation summaries to include writing admission, transfer and discharge orders and referring patients for consultation when indicated (i.E. Degree/Education: Master of Science (MSN) with completion of Nurse Practitioner Program (NPP).
Certifications in addition to Basic Life Support: In addition to BLS;Basic Cardiac Life Support (BCLS) by the American Heart Association;Pediatric Advanced Life Support (PALS) Certification, and Advanced Cardiac Life Support (ACLS) is required for the Critical Care Unit,
Experience: Must have a minimum of two (2) years of experience in medical and surgical applications.

DUTIES OF THE PSYCHIATRIC TECHNICIAN:

The PTA works under the general supervision of the Physical Therapist (PT). The incumbent independently performs day-to-day duties with the supervisor providing general guidelines regarding the nature of therapeutic activities and the program in general. Under the direction of the PT, plans, coordinates, and administers diverse physical therapy programs for wounded, ill and injured service members, as well as other beneficiaries, with a wide range of physical and behavioral health diagnoses and disabilities. Makes recommendations and utilizes the various modalities of physical therapy, as prescribed, to include blood flow restriction, use of cold/heat therapy, diathermy, hydrotherapy, soft tissue mobilization, manual therapy, electrotherapy, ultrasound and therapeutic exercise. Provides updated status in patient progress reports, as well as other administrative reports. Performs other duties as assigned.
Initiates, implements, plans, and recommends progressive treatment plan with consideration of patient s neurological, surgical, sensory, respiratory, cardiac status, gait ability, posture, age and general health, under the direction and supervision of the PT. The treatment plan is designed to restore and maintain maximum range of motion, strength, endurance, coordination, and relaxation in accordance with PT evaluation and treatment plan. Ensures treatment plan is designed to maximize results in promotion of healing and pain relief while ultimately improving the patient s functional independence. Instructs and reviews all home program precautions and contraindications with patients and family members on use and application of physical modalities for home environment. Provides instruction to patients, family members and/or other health workers to perform selected treatment procedures and functional training activities. In the clinic environment, exercises responsibility for the comfort and well-being of patients. In addition to following and executing the treatment plan and objectives, judges whether the treatment is causing an adverse effect on other conditions not currently under treatment. Tracks impact of the procedure s effects on patients, long-and short-term effects of different types of therapy, and appropriateness of a specific therapy, noting overall trends in efficacy, reporting problems to supervisor. Assists physical therapists in monitoring patient progression by measuring range-of-motion, strength, girth of body parts, vital signs, patient perception of pain, and functional status to determine the effects of specific treatments. Reads and executes post-operative protocols per therapist direction and communicate any abnormalities. Shares technical expertise with other therapy assistants, physical therapy students and other interdisciplinary team members. Collaborates with the interdisciplinary team to make recommendations for the patient s rehabilitation and reintegration needs. Assists administrative and clerical duties related to patient care interventions and services provided. Works with clinical and administrative support staff, both above and below the chain of command, in order to accomplish duties. Completes patient documentation in accordance with American Physical Therapy documentation standards, coding guidelines, unit and hospital operating instructions in a timely and thorough manner. Records range-of-motion, strength, girth of body parts, vital signs, patient perception of pain, and functional status as required to track patient progress on established rehabilitation goals. Performs non-technical maintenance of equipment and supplies to prevent malfunctions between patients and at the end of the day. Takes inventory, orders supplies, answers telephone, takes messages, fills out forms, schedules patients and staffs the reception area. QUALIFICATIONS OF THE PHYSICAL THERAPY ASSISTANT (PTA):

Degree/Education: Minimum of an associate s degree from an academic institution accredited by the Commission on Accreditation in Physical Therapy Education is required. A bachelor s degree in a health-related field is preferred.
Certifications: Basic Life Support (BLS)
Licensure/Registration: Current, full, active, and unrestricted license to practice as a physical therapy assistant or possess current certification as a physical therapy assistant as determined by the American Physical Therapy Association (APTA). Employee must maintain current Basic Life Support (BLS) certification IAW AF guidance.

DUTIES OF THE DENTAL ASSISTANT:

Degree: Associate degree or Bachelor degree depending on the program.

DUTIES OF THE DENTAL HYGIENIST:

Examine patient's oral cavity to include mouth, throat, and pharynx, and record conditions of teeth and surrounding tissues. Refer patients to dentist who have abnormalities to include cavities, defective fillings, suspicious growths, or periodontal disease.
Apply desensitizing and other topical agents to treat abnormalities to include caries prevention, gingivitis, and oral ulceration.
Instruct patients, individually and in groups, in proper oral hygiene care using materials to include teeth models, displays, slides, toothbrushes, dental floss, disclosing tablets and mirrors. Demonstrate proper techniques of brushing, flossing, and use of necessary periodontal aids. Explain the common causes of tooth decay and its relationship to general diet.
Take impressions and construct sports/safety mouth guards and soft night guards for vital bleaching. Polish restorations and apply pit and fissure sealants.
Chair side dental assisting when directed by the Officer-In-Charge of the dental clinic.
Record patients treated. Check and maintain instruments to insure working condition. Clean, sharpen, and sterilize instruments.
Patient Workload, Readiness, and Scheduling. Utilizes the Corporate Dental Application to enter patient specific workload and readiness status and also for patient scheduling.

The Diagnostic Imaging Technician is a full-time position, 80 hours per two weeks, schedule can vary. Full benefits, including 17 days paid vacation/sick leave and 11 paid Federal holidays.

DUTIES OF THE DIAGNOSTIC IMAGING TECHNICIAN - MRI:

Performs a variety of MRI/MRA scans of the body in accordance with department protocols and supervising radiologist directions.
Ensures patients are properly screened for safety concerns and/or contraindications for MRI exams. Reports concerns to the supervising radiologist or requested health care provider.
Ensure proper preventive and corrective maintenance for the MRI scanning unit.
Report any patient's need of immediate medical attention. Findings from emergency response shall be immediately reported to the officer in charge of the MR Imaging facility or his authorized representative.
Attend and participate in all meetings, staff conferences, and other appropriate professional activities as required by the supervisor.
Shall assure that all MRI scans are performed in accordance with established protocols and only upon the order of a person who is lawfully authorized to diagnose, treat, and prescribe. Any ambiguous procedures or requisitions will be referred to the director of the MRI facility.
Shall respect a patient's right to refuse the examination. If the patient does not want the procedure performed, the patient's wish shall be respected and the incident logged in the scanner logbook and brought to the attention of the attending radiologist.
Scheduling of MRI / MRA exams and the administrative / clerical functions that are included.
Clerical work that is required for general day-to-day function of the MRI department.
Assist with training new personnel or members with less than 2 years experience but not limited to:
- a. Basic machine and computer console functions
- b. Patient preparation and setup
- c. MRI/MRA scans of the body IAW department protocols and supervising radiologist directions
- d. Scheduling of MRI / MRA exams

QUALIFICATIONS OF THE DIAGNOSTIC IMAGING TECHNICIAN - MRI:

Degree/Education: Graduation from a radiography program accredited by the Joint Review Committee on Education in Radiological Education (JCERT).
Certifications in addition to Basic Life Support: IAW position description / qualifications as stated within Exhibit 5 of the basic contract.
Experience: Radiologic Technologist or Radiation Therapist by the American Registry of Radiologic Technologists (ARRT) or the American Registry of Magnetic Resonance Imaging Technologists (ARMRIT) (ARMRIT authorized after 21 Feb 17).
Have a minimum of one (1) year of experience within the last two (2) years, on MR imaging with any form of MRI equipment.

REQUIREMENTS OF THE RN-PEDIATRICS:

Shall be sufficiently skilled to teach patients proper self-care and to motivate patients and family members to continue the procedures for promoting rapid healing.
Shall be knowledgeable and possess skills necessary to provide care appropriate to any age-related needs of the patients. The Contract RN shall demonstrate knowledge of the principles of growth and development appropriate to the appropriate patient population served: Infants, Children, Adolescent, and Young Adults.
Assess, coordinate, facilitate, deliver and evaluate care for infant to young adult patients using the nursing process.
Knowledge of a wide range of medical disorders/conditions, diagnostic studies and treatments related to Pediatrics, is desirable.
The contracted RN will be knowledgeable in professional care theories, principles, practices, and procedures to perform nursing assignments of moderate difficulty and experience in assessing pediatric patients, as appropriate.
Knowledge of computer operations and proficiency in the use of basic word processing, data entry and automated medical records. Composite Health Care System (CHCS). PYXIS: Ward medications are stored in PYXIS medication stations, access controlled by appropriate individual account access security. Thorough understand of the integration of CHCS/ESSENTRIS/PYXIS with regards to medication dispensation/administration.

Pediatric Chemotherapy Biotherapy course will be completed individually by contract Registered Nurse, while clinical experience in chemotherapy administration will be verified with WRNMMC Department of Nursing Initial/Ongoing Pediatric Chemotherapy Administration Competency Assessment Competencies will be completed at 3W Hematology/Oncology Ward with contractor RN, a designated observer and unit supervisor.
All licensed contract employee will document patient response to PRN medications.
Give inhalation therapy treatments, with knowledge of medications utilized in this treatment and document response to treatment.
RN will start and administer blood product transfusions and be knowledgeable about adverse reactions and institute appropriate measures in the case of adverse reactions, and document patient response to transfusions.
Manage and ensure proper procedures are followed with the tissue/skin bank and blood bank.
Interpret pertinent lab values, notify physician of abnormalities.
Transcribe or verify transcription of physician orders.

Operate and monitor the performance of essential equipment used in the care of stable newborns, e.G. Radiant warmer, blood glucose monitor, pulse oximeter, medications via syringe pumps, etc. Interpret and respond to data generated by such equipment.
Utilize knowledge of normal and high risk maternal, fetal and infant nursing care requirements after delivery to provide anticipatory guidance and education to parents.
Possess knowledge of normal and abnormal newborn physical and neurological development and application of same in delivery of nursing care to infants from birth until seven days of age.
Assess patient readiness for rooming-in, transfer to Neonatal Intensive Care Unit (NICU), or discharge.
Possess knowledge of normal and abnormal female anatomy and physiology, common gynecological surgery and application of same in delivery of nursing care to obstetrical patients.
Safely administer prescribed medications including IV push drugs and IV drips specific to pregnancy, labor and delivery, postpartum and neonatal care as described. Monitor and document patient response.
Administer oxygen therapy, monitor oxygen equipment, and interpret and respond to data. Monitor patient and interpret and respond to data.
Identify potential post-anesthetic patient problems;specifically, airway management, hemodynamic instability and pain control, and intervene appropriately.
Provide BLS and Neonatal Resuscitation Program (NRP) as indicated.
Monitor and transition mothers/newborns during the immediate post-delivery
Knowledge of and/or experience working with intrathecal, epidural, and general anesthesia.

DUTIES OF THE LICENSED PRACTICAL NURSE (LPN) - PATIENT CENTERED MEDICAL HOME (PCMH):

The Contract Licensed Practical Nurse shall perform the following procedures, as required, and in strict compliance with standing orders and protocols set forth by the MTF to include, but will not necessarily be limited to the following:
- Obtain vital signs, procure height and weight, wound irrigations, and suture removal, obtain and prepare specimens for laboratory testing, perform EKG/ECG, perform quality checks and assist with calling patients to remind them of appointments or results as directed by RN or provider.

- Demonstrate competence in starting/performing intravenous (IV) access and phlebotomy/blood draw for specimen collection or present certificate of attendance and or certificate of completing IV and phlebotomy class within 3 months of employment.
- Obtains initial health data in relation to the patient s psychosocial and physiological needs through observing and interviewing techniques.
- Perform on sight screening of patients with a wide range of medical problems.

Degree/Education: Possess an associate, diploma, baccalaureate and master s degree in nursing from a school of nursing accredited in at least one state or territory of the United States. Possess a current active unrestricted nursing license (RN) from at least one state or territory of the United States.
Certifications: Must have current and maintain certification in Basic Life Support Training (BLS).
Experience: Contractor Psychiatric Nurse shall meet the qualifications for that specialty and perform essentially the same functions, within the scope of acceptable for their technical professional discipline and standard, as those required by military and/or government service technical professionals of similar experience and in similar duty assignments. Contractor personnel practices/productivity will be compared to that of other contractor personnel, civil servants and military assigned to the same medical center. The nurse will be caring for child and adolescent psychiatric patients and following skill sets will ensure safe and quality care.
Licensure/Registration: Shall possess a valid, unrestricted license (with no limitations, stipulations or pending adverse actions) in at least one state or territory (Commonwealth of Puerto Rico, Guam or the Virgin Islands) in the United States. Performs administrative duties to include receiving patients and imaging requests, scheduling, and explaining methods of examination in person and over the telephone. Receives and interprets requests or instruction for CT scans. Assesses patient clinical history to determine the most effective protocols. Consults with referring physicians and technicians as necessary to obtain information regarding patient s examination. Utilizes hospital information systems.
Performs daily warm up and assigned quality control checks of the CT scanner.
Coordinates care for routine, emergent, inpatient, and special needs patients with the appropriate clinical staff. Receives patients and explains procedures, providing humane and appropriate care and communicates to alleviate fear of examination. Correctly positions the patient for the examination and makes adjustments necessary for the required examination. Determines if any special patient preparation is required.
Assists/performs consent procedures prior to CT exams requiring written consent from patients.
Operates CT scanner and auto injector. Determines proper instrument technical factors and control settings to ensure correct calculated exposure of targeted body parts. Programs the examination equipment. Sets up the machine and scanning techniques to be employed. Performs all CT examinations according to department protocols.
Assures that sterile supplies, contrast materials, catheters, or other required materials are available and in place. Understands and screens patients for contraindications to contrast use. Starts Intravenous lines for contrast administration. Prepares and administers contrast media orally, by enema, or intravenously under close supervision of a radiologist. Operates contrast power injector. Adjust injection rates. Informs radiologists when a conflict exists prior to injection, and responds promptly to adverse contrast reactions if/when necessary.
Performs a full range of CT examinations including but not limited to;contrast and non- contrast, head, neck, chest, abdomen, pelvis, spine, extremities and specialized studies such as angiography, etc.
Performs numerous complex CT examinations on all anatomical areas for neonatal, pediatric, adolescent, adult and geriatric patients. Assists radiologists and/or other physicians in performing invasive diagnostic techniques such as biopsies, aspirations, radiation therapy and interventional procedures.
Adjusts imaging parameters to include: window/levels, sets field of view, obtain part measurements, obtains regions of interest, reconstructs, and reformats images. Performs digital reconstruction of acquired images in accordance with standard protocol, physician request, and radiologist needs. Performs 3D reconstruction as appropriate to physician needs.
Ensures only optimal examinations are presented to the radiologist for interpretation. Maintains records of patients examined, examinations performed, sequences taken, and technical factors used.
Identifies abnormalities during testing and determines need for additional scans of affected area. Notifies physician of significant scans requiring immediate attention.
Utilizes the hospital information system and Picture Archiving Computer System (PACS) to acquire, transmit, and store digital CT examinations.
Images to be read remotely will be transmitted by the technologist, according to department policies, utilizing PACS.
Routinely performs digital image library functions to include acquiring, reformatting, transmission, storage, and loan procedures.
Maintains and cleans CT equipment on a regular basis in accordance with Radiology department standards. Stocks and maintains an adequate level of supplies required to perform CT examinations. Recommends necessary monthly repairs when diagnostic quality is degraded.
Differentiates artifacts from normal or pathological processes and recognizes electronic equipment limitations.
Practices safety principles. Performs two patient identifier verification of patients. Assists with consent procedures prior to exams requiring written consent from patients. Safely oversees, and brings to practice, As Low As Reasonably Achievable (ALARA) principles for patients and staff. Performs equipment quality control checks. Monitors personnel to ensure protective procedures such as radiation safety, hazardous material communications, and Air Force Occupational Safety and Health programs are followed. As needed, assists with student training activities IAW with regulation and course director plans;may instruct new personnel as they observe standard procedures to complete on-the-job training.

QUALIFICATIONS OF THE DIAGNOSTIC IMAGING TECHNICIAN - COMPUTED TOMOGRAPHY:

Degree/Education: Graduation from a radiologic technologist training program accredited by the Joint Review committee on Education in Radiological Education (JCERT).
Certifications in addition to Basic Life Support: o Have and maintain current certification in Basic Cardiac Life Support (BCLS) by the American Heart Association. In addition to BCLS, 17 certification in Advanced Cardiac Life Support (ACLS) is required for the Critical Care Unit.
Experience:
- Must be knowledgeable in anatomy and physiology;medical terminology and ethics;legal aspects of medicine;healthcare accreditation standards;radiation physics, radiobiology, and radiation protection;basic electronics theory;techniques of operating radiographic and specialized diagnostic imaging equipment;radiographic positioning;patient care and monitoring techniques;image recording media and processing techniques;quality control procedures;infection control;aseptic and sterile techniques;reactions to contrast media;BLS;methods of recording the fluoroscopic image;and medical records administration.
- Have a minimum of one (1) year of experience within the last three (3) years, as a Computed Tomography technician. Experience must be as a registered technologist. Any experience prior to the member s ARRT certification date is discounted.

The contractor shall support treatment to the ECC, ambulance emergency response, in-flight emergency response, non-in-flight emergency response, transfer and transport services for active duty military personnel, their dependents, retirees, their dependents and other eligible beneficiaries, in accordance with the terms and conditions of this agreement. The Paramedic will be required to assist with patient care within the ECC when not performing emergency response or transport services. Paramedic shall cover the range of services provided in a civilian medical treatment facility. Paramedic shall provide the following, but not limited to:
- Responsible for creating, maintaining, and disposing of medical documentation.
- Performs a full range of emergency room support and ambulance services on site at the MGMCSC and in the field when performing ambulance emergency or in-flight response, transfer, or transport services. Provides emergency hospital and pre-hospital care for all types of patients (all ages).
- Assesses, plans, implements, and evaluates nursing care for patients of all ages. Workload includes the evaluation and treatment of emergent, urgent, and non-emergent patients ranging from infants to elderly.
- Assisted by medical technicians, the contractor s personnel will be fully integrated into the nursing team, responsible for direct patient care.
- Recognize medical emergencies and responds appropriately.
- The EMT-P will be required to assist with patient care within the ECC when not performing emergency response or transport services.
