Job Description
Currently recruiting a Registered Nurse (RN) Referral Manager to work in the Medical Management Department of Naval Medical Center Portsmouth in Portsmouth, Virginia. This is a full-time position providing care to military Active Duty heroes, their families, and retirees Full-time, 40 hrs/wk, Mon - Sat between 7AM - 7PM.

DUTIES OF THE REGISTERED NURSE REFERRAL MANAGER (RN):
- Perform a full range of RN duties, including: triage; patient assessment and monitoring; appropriate nursing care, procedures, and treatments; execution of physicians orders; documentation of patient care and observations; and patient education and emotional support.
- Assist in the design and implementation of activities to increase staff involvement and support of an active and coordinated medical management program. Work in coordination with case management, utilization management, disease management and other members of the health care team. Primary duties shall entail provision of care and treatment of individual patients.
- Provide review and evaluation of patient case management referrals within 24 hours of receipt to the medical management department.
- Review and evaluate individual patient referrals and assist in the establishment of on-going beneficiary care within the MTF established network.
- Ensure the patient s primary care chart is up to date with information on specialist consults, hospitalizations, ED visits and community organization related to their health.
- Serve as the system navigator and POC for patients and families, with patients and families having direct access for questions and concerns.
- Operate and manipulate automated systems such as CHCS, AHLTA, ADS, Essentris, and Clinical Information System (CIS).
- Clinically direct the review and assignment of cases when directed by the supervisor to appropriate case managers based on type, location, acuity, etc.
- Alert physicians to significant changes or abnormalities of patients conditions, medical history and specialized treatment plan or protocol.
- Ensure policies and procedures, workflow, filling and reporting systems remain current as they relate to home health care and management of durable medical equipment.
- Assist with durable medical equipment and serve as a liaison for home health patient referrals.
- Degree: Associates Degree of Nursing.
- Education: Graduate from a college or university accredited by Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). An alternative to this is to graduate from a state accredited professional nursing program.
- Possess a minimum of two years of experience as an RN with at least one year of experience in referral management, utilization management/review, home health, or discharge planning within the last three years.
- Possess and maintain a current unrestricted license to practice as a registered nurse in any state, the District of Columbia, the Commonwealth of Puerto Rico, Guam, or the US Virgin Islands.
- Must be a U.S. Citizen (for access to Gov' t computer systems)
- BLS is required.
