Job Description
Currently recruiting a Nurse Practitioner (PT Care Coordinator) to provide perioperative and surgical care coordination services at Naval Medical Center San Diego in San Diego, California. This position supports complex surgical and procedural patients across inpatient and outpatient settings, with close coordination among surgeons, clinical staff, and multidisciplinary care teams. Services are provided in support of the Main Operating Room and surgical services in a military medical center environment.
DUTIES OF THE NURSE PRACTITIONER (PT CARE COORDINATOR):
- Provide a full range of Nurse Practitioner services in accordance with privileges granted by the Military Treatment Facility.
- Provide general screening, medical care, and examinations for routine, acute, and chronic conditions.
- Diagnose, treat, and counsel patients as clinically indicated.
- Diagnose and treat patients with common acute conditions, chronic illnesses, or minor trauma within accepted protocols, clinical practice guidelines, Nurse Practice Acts, and physician consultation as appropriate.
- Manage complex perioperative patients in inpatient and outpatient settings.
- Support perioperative optimization, postoperative management, and complication recognition.
- Coordinate care for multidisciplinary and high acuity patient populations.
- Collaborate closely with surgeons, nurses, corpsmen, and multidisciplinary clinical teams.
- Provide clinical guidance to members of the healthcare team in the coordination of patient care.
- Order and interpret diagnostic studies and prescribe medications in accordance with institutional policy and scope of practice.
- Support care coordination, case management, and clinical program operations.
- Contribute to efficient clinic and surgical service operations.
- Oversee or mentor clinical support staff as appropriate.
- Participate in quality improvement initiatives, clinical pathway development, and outcomes tracking.
- Promote preventive care and health maintenance, including positive health behaviors and self care.
- Communicate and collaborate with patients, families, and healthcare team members regarding plans, actions, teaching, and education.
- Recognize conditions that require isolation and ensure universal precautions are used in patient encounters.
- Attend staff meetings for case discussion, continuing education, continuous quality improvement, and safety.
- Perform timely, accurate, and concise documentation of patient care.
- Operate and document in required automated systems, including MHS GENESIS, as applicable.
- Comply with infection control, safety, patient privacy, documentation, and facility specific policy requirements.
QUALIFICATIONS OF THE NURSE PRACTITIONER (PT CARE COORDINATOR):
- Master s or Doctoral degree in Nursing from an accredited Nurse Practitioner program required.
- Graduate from an academic program within the specialty field of nursing study accredited by a regional or national nursing accrediting agency recognized by the Accreditation Commission for Education in Nursing or the Commission on Collegiate Nursing Education.
- Current, active, valid, and unrestricted license to practice as a Nurse Practitioner in any U.S. Jurisdiction required.
- Must be eligible for hospital credentialing and privileging.
- Minimum of 5 years of advanced practice provider experience required.
- At least 3 years of experience in a surgical or procedural subspecialty required.
- Experience in maxillofacial surgery, ENT, plastic surgery, trauma, or head and neck oncology preferred.
- Demonstrated experience managing complex perioperative patients in inpatient and outpatient settings required.
- Experience coordinating care for multidisciplinary and high acuity patient populations required.
- Ability to independently assess, diagnose, and manage patients within scope of practice.
- Experience with perioperative optimization, postoperative management, and complication recognition.
- Proficiency in ordering and interpreting diagnostic studies and prescribing medications in accordance with institutional policy.
- Experience in care coordination, case management, or clinical program support.
- Demonstrated ability to collaborate closely with surgeons and multidisciplinary teams.
- Experience overseeing or mentoring clinical support staff and contributing to efficient clinic operations.
- Participation in quality improvement initiatives, clinical pathway development, or outcomes tracking preferred.
- Strong clinical judgment, communication, and organizational skills.
- Ability to function with a high degree of autonomy while maintaining collaborative practice.
- Proficiency with electronic medical records and digital health tools.
- Must be able to complete required credentialing before providing services.
- Must be able to complete required health, immunization, tuberculosis, and Hepatitis B screening requirements before providing services.
- Must be able to complete required background/security processes, including fingerprinting, for access to a Navy and Defense Health Agency medical facility.
- Must be a U.S. Citizen for access to U.S. Government networks.
- Must comply with all applicable facility policies, procedures, training, safety, infection control, documentation, and patient privacy requirements.
